Transition from Children's to Adult Services

At MAM Care, we know that moving from children’s to adult services can be overwhelming — it’s close to our hearts because we understand the difficulties families face. Transitions often shift from holistic care to task-based care, which can reduce independence and family involvement, adding stress at an already challenging time. MAM Care’s approach is designed to minimise disruption and stress, supporting young people aged 15–25 so families only have to transition once, smoothly, and with the knowledge and confidence needed to make decisions about their care. Our team provides specialist, person-centred support, working closely with families, professionals, and adult care services to maintain continuity, reduce anxiety, and empower young people to take an active role in their care and promote independence.

What Transition Support Looks Like

Our transition support is centred around getting to know the young person and their family first, ensuring any changes happen gradually, safely, and with the right support.

Our approach may include:

  • Early introduction to MAM Care – where possible, we begin working alongside the existing provider, introducing a small number of shifts with our team so the young person and family can slowly get to know us and build trust.

  • Appointing a dedicated lead nurse – a clinical lead is assigned to guide and oversee the transition, ensuring the young person’s needs are fully understood and supported.

  • Getting to know the young person first – before formal transition planning begins, we take time to understand the young person’s personality, routines, preferences, and family dynamics. This ensures we continue to support their life in a holistic and meaningful way, not just their medical needs.

  • Working closely with adult services – we liaise with adult service providers and healthcare professionals early to ensure the transition is smooth and well prepared.

  • Supporting families during clinical changes – if the move to adult services requires changes in equipment or clinical care (such as feeding tubes or tracheostomy care), we support families through meetings with professionals so they feel informed and confident.

  • Flexible care packages – care is adjusted only where needed, ensuring the package continues to meet the needs of the young person and their family.

  • Ongoing family guidance and reassurance – we remain available throughout the process to provide support, answer questions, and help families navigate the transition.

Every transition package is clinically led and carefully monitored. Our in-house team of children’s and adult service leads ensures carers are fully trained, competent, and supported to deliver care safely. This guarantees the young person’s needs remain at the centre of care, while families feel confident that quality and safety are maintained throughout the transition.
